Position: Market Medical Director — Florida
Reports To: Sophia Warren – Market President
Location: Tampa, FL, Jacksonville, FL and South Florida -Remote-First with in-market travel
Employment: Full-Time
Travel: 25–40% across Tampa Bay, Jacksonville, and South Florida
On-Call: Participates in clinical leadership coverage; no traditional overnight call
The Opportunity
This is a physician-leader role — not a purely administrative one. Duo Health expects you to hold both the bedside and the boardroom with equal confidence, splitting time between direct patient care (home-based/community) and clinical leadership responsibilities across the Florida market.
As Market Medical Director, you will serve as the senior clinical voice for Florida, overseeing three geographically dispersed markets:
Market: Tampa Bay (HQ Market)
Geographic Coverage: Lutz, Port Richey, south to Sarasota
APPs: 3
Travel: Home-base
Market: Jacksonville
Geographic Coverage: Northeast Florida
APPs: 2
Travel: Every 6 weeks
Market: South Florida
Geographic Coverage: Jupiter / West Palm Beach south to South Miami
APPs: 5
Travel: Monthly
Key Responsibilities
• Serve as the senior clinical voice for the Florida market across Tampa Bay, Jacksonville, and South Florida
• Lead, coach, and develop 10 APPs across three geographically dispersed teams
• Drive hospitalization reduction, HEDIS/STARs performance, and HCC documentation quality
• Build and sustain trusted relationships with community nephrologists and nephrology practice partners
• Engage in direct patient care (home-based/community) with a small panel of high-risk CKD/ESKD patients
• Contribute clinical perspective to product, technology, and payor contract decisions
• Own QAPI and chart review processes for the Florida market
• Partner with the Market President and CMO on market strategy and team development
What You’ll Need
• M.D. or D.O. with board certification in Internal Medicine or an Internal Medicine subspecialty
• Active Florida medical license (or ability to obtain)
• 7+ years of clinical practice experience, ideally including outpatient or home-based care
• Experience in clinical leadership roles (e.g., Medical Director, Service Line Leader, Division Chief)
• Track record coaching and developing APPs and other clinicians
• Experience using KPIs and data to improve quality outcomes and manage cost of care
• Fluency in value-based care principles: risk adjustment, HEDIS, utilization management
• Experience with advanced kidney disease and ESKD clinical management strongly preferred
• Entrepreneurial mindset and comfort in a high-growth, start-up healthcare environment
• Based in or near Tampa, FL, Jacksonville or South Florida

Former President of the National Kidney Foundation (2006–2014). Deeply fluent in ESKD care models, ESCOs, C-SNPs, and value-based kidney care contracting.
Sophia Warren, NP, MS — Florida Market President (Direct Supervisor)
Accomplished healthcare executive and nurse practitioner based in Sarasota, FL. Former VP of Patient Access Expansion at WellBe Senior Medical. Deep expertise in clinical program leadership, utilization management, quality improvement, and scaling telemedicine models across complex senior patient populations.
Brie Farmer — President & COO
Most recently COO of WellBe Senior Medical. Prior to that, SVP of Customer Onboarding and Integration at R1 RCM. Focused on scaling and systematizing Duo’s operations as the organization continues to grow its reach across markets and payer partners.
