About the program
Omron operates a national, technology-enabled hypertension care program serving Original Medicare beneficiaries. Care combines cellular-connected home blood pressure monitoring, clinician-led medication management, lifestyle coaching, and care coordination with each patient's primary care practitioner.
Under ACCESS, a 10-year voluntary CMS model running July 2026 through June 2036, participants receive fixed, outcome-aligned payments over 12-month care periods. Full payment depends on patients reaching blood pressure targets, such as a 15 mmHg systolic reduction or control below 130/80 mmHg. CMS requires every participant to designate a Medicare-enrolled MD or DO as Medical Director, and allows that role to be filled by contract.
Key responsibilities
- Clinical policies and patient safety
- Develop, review at least annually, and approve clinical policies and procedures for ACCESS care, consistent with current AHA/ACC hypertension guidelines.
- Own the written patient safety plan: protocols for identifying and reporting safety concerns, care escalation pathways, and management of adverse events.
- Approve clinical exclusion criteria and protocols for transitioning patients whose needs exceed program scope.
Clinical oversight
- Provide oversight of all physicians, nurse practitioners, physician assistants, pharmacists, and nurses furnishing ACCESS services.
- Approve medication titration protocols and standing orders; serve as escalation point for complex or high-risk cases.
- Ensure qualifying-condition validation and informed consent processes are followed before enrollment.
- Oversee clinical use of connected devices and any FDA-regulated software in care delivery.
Outcomes, data, and quality
- Monitor patient outcomes against ACCESS blood pressure and cardiometabolic targets, and guide corrective action plans.
- Reasonably validate outcome measure data submitted to CMS, including source-verified blood pressure readings.
- Review quality, safety, and overtreatment indicators; chair a periodic quality committee.
Compliance and coordination
- Ensure adherence to federal and state law, including licensure, scope of practice, HIPAA, and ACCESS Participation Agreement terms.
- Serve as the named Medical Director on care plans shared with patients' primary care practitioners.
- Support CMS audits, monitoring requests, and the independent model evaluation.
Required qualifications
- MD or DO degree.
- Active, full, unrestricted license to practice medicine in at least one U.S. state.
- Individually enrolled in Medicare and in good standing, or able to complete enrollment before Omron executes its ACCESS Participation Agreement.

Preferred qualifications
- Multistate licensure. Strong preference for physicians eligible for, or holding licenses obtained through, the Interstate Medical Licensure Compact (IMLC), or holding active licenses in a large number of states. Candidates should list all active state licenses.
- Willingness to obtain additional state licenses as [Company Name] expands; licensing fees reimbursed.
- Hypertension specialist certification (American Hypertension Specialist Certification Program) or obesity medicine board certification (ABOM).
- Prior medical director or supervising physician role in a telehealth, remote patient monitoring, or chronic care management program.
- Experience with CMS Innovation Center models, value-based contracts, or outcome-based quality reporting.
